Experience: We ask that you please list all of your trailer hauling experience. However, understanding your current level of experience will allow Horizon to know how much training we will need to provide to get you started.
Orientation Preparation:

1. Applicant application requiment I fully understand that when I attend orientation I will be asked to complete and finish the formal application process. The application will be available to you at the orientation location. However, if you wish to download and complete the application prior to arriving, please click on the download application link from your confirmation page.
2. Social Security Card All contractors must present a Social Security card for our files. If you do not possess a card you must apply for one prior to orientation and bring the application receipt to orientation.
3. Driver's License We will need to take a photo copy of your current state driver's license for our driver file.
4. Vehicle Registration You must present a current state registration for your vehicle. We will take a copy and place it in your file. However, we do not need to know if you own or lease your vehicle. My vehicle is registered in my name You must present a current state registration for your vehicle. We will take a copy and place it in your file. My vehicle is not registered in my name Horizon will need a power of attorney signed by original owner and notarized by both you and the original owner. 5. Vehicle Insurance Information You must bring a copy of your insurance card and certificate showing your policy coverage and effective dates.
6. DOT Physical May use existing DOT physical if it has been administered within the previous 6 months of your application date. Applicant will be responsible for having physical completed prior to attending orientation.
7. Transport plate deposit Contractor will be assigned transporter plates during orientation. The refundable deposit requirement is $50.00 for two plates.
8. Door signs Contractor will be assigned 2 magnetic door signs to be magnetically affixed to the sides of the truck. Cost of signs is $20
9. Standard Receiver Hitch, Rear Mounted Your vehicle must also be equipped with a receiver hitch with a minimum rating of 10,000 pounds and a minimum 10K rated stinger.
10. Torque Wrench with rating of at least 120 pounds (to check lugs at beginning, middle, and end of trip.)
11. All Electrical Connections Be prepared to connect to trailer lighting from your towed vehicle.

13. Trailer Hitch Insert (a.k.a. a "stinger") and (2) hich-balls (2" and 2 5/16")
