Driver Qualifications
- Must be 25 years of age

Towing Experience: Horizon Transport Towaway Drivers must have a minimum of 20,000 miles towing experience.
- CDL Requirement
Each state may differ regarding what minimum license class you should possess. You must check with your state DMV and let them
know that you are desiring to use your truck to deliver commercial trailers, "what license should I have?"
- Acceptable MVR- Reports will be checked for the previous 3 years. It cannot contain more than 1 moving violation or a total of 4 points
- No moving violation suspensions
- No history of drug, DUI, DWI, OWI, careless driving, reckless driving or habitual offender convictions
- Full 10 year work history
You must document a full 10 year work history on your application. If you have any gap of more then 60 days
between jobs you must submit $50.00(cash or money order only) in order for us to run a background check. If
you are retired, owned your own business, or have more than 4 work references in the past 3 years you must
submit a professional letter of recommendation. What is a professional letter? click here
- Pass a company road test
- Pass a DOT Drug test
Pre lease drug testing must be done in Wakarusa on day 1 of orientation.
The cost is $45.00
- Pass a DOT Physical
May use existing physical if it has been administered within the previous 6 months of your application date.
Physical testing can be done locally for $45.00
- Must have a home number
Must have a cell number
We ask that you come to orientation with an active cell phone and voice mail established.
- $1000 Damage Deposit
Each qualified contractor is responsible to pay damage deposit during the orientation period.
Methods of payment
1. Personal check, money order or cash.
or
2. You may have 10% of each trip deducted until you attain the $1000 level.
Footnote: In the event that a contractor de-leases, prior to completing 25 loads, a $250
administrative fee will be charged against the deposit
- $50.00 Plate deposit
Each contractor will be assigned a transporter plate at orientation. You will be required to submit
a $50.00 deposit for your plate.

Truck must be a full size
¾ or 1 ton (dually not necessary) No mid size trucks or vans
Horizon Transport
will not lease on any truck bed under 8 foot long. Your truck will be
measured at orientation and must meet our 8 foot standard length.

Must submit a completed DOT
inspection
Inspection can be done at the following location:
Dan's Service Center- Elkhart, IN, D&M
Automotive or Monteith Tire, Wakarusa, IN
Note: Before a DOT truck inspection is administered, you must have an
emergency triangle kit and a mounted fire extinguisher with a gauge in your truck.

truck, the bobtail is above this standard coverage.

Truck model must be 5 years
old or newer.
No older than a 2003
Reminder: This means you may come aboard with a 2003 or newer vehicle and you will not be required to upgrade the year of your truck as long as it remains leased to Horizon Transport. You are not required to upgrade the truck, once it is leased on, even after it is ifve model years old or older.

Truck must be owned. If your truck is
under a lease we will not process your application.

Truck must be equipped with

a) Class III receiver
Rated at least 10,000 pounds
b)
5th wheel
Rated at least 20,000 pounds
c)
Gooseneck hitch
d) Full width mud flaps at rear bumper and wheel flaps
e) Extended mirrors for 102 inch trailers
f)
1,000 pound load distribution bar
g) Credit card to do your own truck repairs (on the road)
h) Torque wrench-rated at 125 lbs
2 deep well 6 point socket sizes:
13/16 &

i) Electric brakes
j) Appropriate bed plug and bumper plug
k) (4) 27 foot tie down straps 2inches wide. These are used for securing
stacked loads.
l) 2 ball sizes: 2" and 2 5/16

DOT requires the battery to be

"wet" cell battery not a dry cell
